Utilizing our Patient Portal in this way will make your visit more effective by allowing us to concentrate on your health instead of taking up time with clerical work.

If, for any reason, you are unable to enter this information, please phone us at least two days prior to your visit and allow our nursing staff to enter it for you.

Using our Patient Portal to complete and update your information helps us prepare for your appointment and keep the most accurate record of your medical information. It has been shown that completion of the records in the comfortable atmosphere of your home allows for more complete recollection of the medical events and history. It is now our request that all of the information pertinent to the upcoming appointment gets completed online prior to the appointment if possible.
